Checklist

What comes in the kit • 3 SST Tubes • 1 Lavender Tube

• 1 Royal Blue Tube Red • 1 Green Lithium Tube

• 1 Amber Tube • 1 Urine Cup

• 2 Pipettes • Tube Labels

Tubes to draw 7 Tubes in total

• Draw 3 SST Tubes first - fill to top; • Draw 1 Lavender Tube - fill to top;

• Draw 1 Royal Blue Tube - fill to top; • Draw 1 Green Lithium Tube - fill to top.

Processing & Centrifuge

Centrifuge - for tubes that require centrifuge: centrifuge for at least 15 minutes • Centrifuge 3 SST Tubes; • Centrifuge 1 Green Lithium Tube - Plasma;

• Centrifuge 1 Lavender Tube. Pipette plasma into Amber Tube. Check the box on the label “Plasma from Lavender“ and label this tube with patient Name and DOB.

Place 1 Amber Tube and 1 Green in the freezer for a minimum of 15 minutes and then place in provided freezer bag.

Refrigerate & Freeze samples

Please refrigerate samples after collection and processing • Refrigerate 3 SST Tubes, 1 Green Tube, 1 Lavender Tube, 1 Royal Blue Tube;

• Freeze 1 Amber Tube.

Shipping checklist What gets sent to the lab

• Frozen ice pack - lay flat on bottom and top of cooler box; • All tubes placed within provided specimen bag (3 SST Tubes, 1 Lavender Tube, 1 Green Tube, 1 Royal Blue Tube, 1 freezer bag w/ Amber Tube, 1 Urine Cup);

• Patient requisition form; • Copy of patients insurance;

• Make sure all tubes are clearly labeled with Patients Name and DOB • Requisition form and copy of patients insurance must be placed with specimen.
