MESSAGE From our CEO, Carlos de Solo Dear Team, I am writing to share exciting news for our company. This morning, we announced an agreement to acquire the Medicare value-based care business of Steward Health Care System in a cash and stock transaction. This is a truly transformative step in the continued growth and evolution of our company, and one that we believe will position us to help redefine healthcare delivery in the U.S. You can read the press release that we issued here:

CareMax, Inc. to Acquire Medicare Value-Based Care Business of Steward Health Care System For those of you not familiar with Steward Health Care, they are a nationally recognized, fully integrated healthcare system, operating 39 hospitals across nine states. CareMax has agreed to acquire Steward’s value-based care network, which spans approximately 170,000 patients and 1,800 providers. In addition, Steward and its affiliated practices service approximately 387,000 Medicare Advantage Fee for Service patients and 482,000 Traditional Medicare Fee for Service beneficiaries. Following the close of the transaction, CareMax’s network will expand to approximately 2,000 providers and reach over 200,000 senior value-based care patients across 10 states. This transaction builds on our decade of innovation and accelerates our vision to bring our model of whole person health care to more communities than ever before. With the addition of the Steward value-based care business, CareMax will become a leading independent national Medicare value-based care platform. While this is an exciting and transformative moment for CareMax, please keep in mind that today’s announcement is just the first step in a process. There are approvals to obtain and customary conditions to meet before the transaction officially closes, which we expect to occur late in the third quarter or early in the fourth quarter of 2022. Furthermore, this will have no impact to the day-to-day operations of our physicians, nurses, administrators, clinic teams, operations staff and over one thousand employees that we have working to deliver healthcare with heart to the communities that we serve. EMERGENCY PREPAREDNESS Don’t wait. Communicate. Make severe weather planning and preparedness a priority! CareMax recognizes that not all inclement weather or natural disaster events are foreseeable. Therefore, appropriate planning, preparation, response, and recovery are essential to maintaining the safety and well-being of both our team members and our patients during an event. In the event of an emergency the company’s Emergency Preparedness and Response Team (EPRT) will inform you about what to expect before, during, and after an emergency. Elements addressed in this plan include employee responsibility, communication, safety, evacuation plan, sheltering, special needs shelters, and more. Preparation remains the most important step we can take to protect ourselves and our families. Hurricane Season | June 1 - November 30 Hurricane Safety Tips

▪ A disaster supplies kit (first aid kit, water, canned foods, etc.) ▪ Bring in anything that can be picked up by the wind ▪ ▪ ▪ ▪

(bicycles, lawn ornaments, patiofurniture, etc.) Close windows, doors, and hurricane shutters. Fill your vehicle’s gas tank and any gas tanks you may have. Obey evacuation orders. Pet owners should have plans to care for their animals.

Tornado Season | Although tornados are most common during the spring months of March, April, and May, severe weather and flooding can occur any time. Tornado Safety Tips Prevention and practice before the storm: At home, have a family tornado plan in place, based on the kind of dwelling you live in and the safety tips below. Know where you can take shelter in a matter of seconds and practice a family tornado drill at least once a year. At Home ▪ Go to basement or smallest center room ▪ Stay away from all windows ▪ Cover yourself - protect your head. High - Rise

EMPLOYEE SPOTLIGHT Odalis Gonzalez Ramirez Odalis Gonzalez Ramirez loves her job. As Market Development Manager at CareMax, Odalis oversees ensuring continued growth and retention, but her work is much more than that. “What I do is not a job, for me it is a service of love,” said Odalis. “My days go flying by. I wish I had more hours to continue doing my work.” Odalis grew up in Ciego de Ávila, Cuba, the eldest of three sisters. When she was eight years old, her mother became ill. From that point on, Odalis helped raise her sisters. She also became a voracious reader. “I remember finding a phrase that struck me,” said Odalis. “It said something like ‘be like a candle, which is consumed giving light to others.’ I grew up with that life motto.” Instead of being bitter about her situation, Odalis thrived in it. “I did everything to help my mother, my sisters, my grandmother, the little ones, everyone who needed it,” she said. “At the same time, I became extroverted, sociable, and more determined to be a leader.” Odalis worked in many professions in Cuba. She was a librarian, a teacher, a TV producer, a radio scriptwriter, a health promoter, and a public relations specialist. She even earned a master’s degree in Education. Every job, every chance she had to learn, gave her another piece of the puzzle that would eventually come together as her career. Odalis immigrated to the United States in 2014

Odalis came to DNF Medical Center as a patient but fell in love with the company. Working there became her new goal. In 2016, she became a DNF employee. She finally realized all her experience, everything she learned from the time she was eight, led up to a profession that was also her passion. After the acquisition, Odalis stayed on and became a CareMax employee. “I am happy, very happy, with everything I do. Every day I learn something new,” said Odalis. “I have had the opportunity to build long-lasting and valuable relationships with our patients. In each one of the seniors I work with, I see my grandmother, my mother, and myself in the future. Every day I am more convinced God put me here for a mission of love. “ Odalis considers her work at CareMax to be a lifelong career. “I always try to give the best of myself, to grow as a professional,” she said. “It is an honor and a privilege for my work to be recognized.”

RECOGNIZING JUNETEENTH DAY Juneteenth marks the end of slavery in the United States. Our fellow CareMax team member, Michelle Cyrus-Brown, gives us a little insight into this important day and why we at CareMax recognize this historic moment. What does Juneteenth mean to you? Juneteenth allows me to reflect on the day the final emancipation of African Americans from the system of slavery in the United States was able to happen.

Do your family or friends share your opinion on it? Absolutely. It is a part of the culture of African Americans. As such, it is embraced as a significant part of the history of the country.

Do you have a fond memory related to this day? I once attended a Juneteenth celebration in Atlanta during a visit. The cultural celebration was beautiful with school marching bands, food, and just plain fun.

Why is it important for others to know the meaning of Juneteenth day? I believe all aspects of the history of the United States should be understood, and we should know the significance of events that took place.

What message would you want your coworkers to know about this day? Juneteenth is a special day for African Americans. It is a celebration of freedom, culture, and achievements.

Is there anything else you want to share? I am happy and proud to see CareMax taking opportunities to embrace the importance of culture and diversity. CAREMAX | JUNE 2022

MEN’S HEALTH MONTH Message from Dr. Pablo Alonso

June is National Men’s Health month, the perfect time to increase awareness and encourage men of all ages to take care of their health.

Men are less likely to seek medical attention than women. According to the Centers for Disease Control and Prevention, men in the United States, on average, die five years earlier than women. They also die at higher rates from the three leading causes of death: cardiovascular heart disease, cancer, and unintentional injuries. Men should be encouraged to visit their primary care provider at least once a year to identify risk factors that, if not addressed in a timely manner, could lead to serious disease and complications. Important tests to include are: ▪

Diagnostic tests such as hemoglobin A1c to screen for diabetes



Lipid profiles to determine cholesterol and triglyceride levels



HDL (good cholesterol) and LDL (bad cholesterol) levels to guide in preventative treatment for heart disease and stroke



PSA to check for prostate health



Blood pressure measurements to identify if treatment is necessary (this will also minimize the risk for cardiac disease and stroke)



Screening for depression

While the focus this month is on men’s health, it is important to remember there are things everyone can do to get or stay healthy. Exercising a minimum of three times a week is recommended. Additionally, everyone should practice good nutritional habits by eating a balanced diet with an abundance of vegetables and fruits, while also avoiding fats, excess carbohydrates, and alcohol. To keep spiritually healthy, engage in hobbies, activities and socialize with friends and family. CAREMAX | JUNE 2022

WHAT IS THE CAREMAX CUP? By Mark Llorente, President, South Region

The CareMax Cup is a quarterly initiative focused on improving center performance and integrating the CareMax culture across all centers. By creating teams across different markets, the initiative promotes the sharing of best practices, team building and accountability with the goal of improving member retention and the growth of each center. The teams ranking in first, second and third place are announced monthly, and the CareMax trophy is temporarily being housed at each of the first-place centers. At the end of the quarter, the winners of the quarter are announced, and each winning center is rewarded for their contributions. The excitement and enthusiasm created by the CareMax Cup initiative has impacted our team members and patients alike. Celebrations at the winning centers, including the participation of some of their members, has spurred friendly competition between center teams and inspired all center level staff – from drivers and janitors to receptionists and clinical staff. As a result, performance at the centers has significantly improved, as our new and existing team members work together to provide “healthcare with heart.”
