What’s New for 2021-2022? ✓

Summit will continue to offer the same two medical plan options with BlueCross BlueShield of Texas and Prime Therapeutics. . You may select either the Health Investment Plan or the PPO Plan. The Health Investment Plan option may be paired with a Health Savings Account which will provide you an opportunity to invest in your health today and in the future.



There will be no change to employee premium contributions this year. Summit continues to pay the majority of your healthcare expense.



For employees who enroll in the Health Investment Plan, if you enroll in the Health Savings Account, Summit will continue to contribute money into your HSA. • $500 for employee only • $1,000 for an employee plus one or more ❑ To receive employer contribution, you need to open an account with Wex (formerly Discovery Benefits) if you have not already AND elect to make a payroll deduction and fund any amount into your account.



Dental and Vision benefits will remain the same.



Summit continues to offer a telemedicine benefit through Teladoc so that those members enrolled in one of the medical plan may have access to providers without visiting a doctor’s office. Enrolled dependents are also eligible.



Cigna life and disability coverage is changing its name to New York Life. You may see both names this year. For all employees currently enrolled under the voluntary life coverage through Cigna/New York Life, you may increase your benefit by one Benefit Level ($10,000), as long as your total Benefit does not exceed the Maximum Benefit, without satisfying the Insurability Requirement. A request to increase your spouse’s voluntary life Benefit can be made by submitting an Evidence of Insurability form. You may also add short- or long-term disability coverage without having to submit a health questionnaire/Evidence of Insurability form.

Review your 2021-2022 Benefit Plan Enrollment Guide online at UKG. If you need assistance or have questions during Open Enrollment, please contact Benefit Harbor at 888-408-5654. They are available Monday-Thursday from 8-8 cst and Fridays from 8-6 cst. Gather information and documents required for enrollment, including dates of birth, social security numbers of dependents and required dependent eligibility verification documentation. When deciding on the best medical option, think about your health history and how things might change in the upcoming year. Thinking about the services you need can help you determine what type of coverage is right for you. All eligible employees must log in to UKG and select “Stay enrolled in Current Benefit Plans” to keep current selections or select “Make New Elections” to make changes to current selections and go through the Open Enrollment process for your 2021-22 benefits. Current elections do not automatically roll forward – you must make the election in UKG. Please note: If you are enrolled in the HSA or FSA and wish to contribute to the account for 202122, you must use the “Make New Elections” section in UKG.
