Choose Carefully!
You are eligible for benefits if you work 30 or more hours per week. You may also enroll your eligible family members under certain plans you choose for yourself. Your eligible family members include:
Due to IRS regulations, you cannot change your elections until the next annual Open Enrollment period, unless you have a Qualifying Event during the year. Following are examples of the most common Qualifying Events:
Your legally married spouse Your Registered Domestic Partner (RDP) or their children, where applicable by state law Your children who are your natural children, stepchildren, adopted children, or children for whom you have legal custody (age restrictions may apply). Disabled children age 26 or older who meet certain criteria may continue on your health coverage. Required Information — At enrollment, you are required to enter the Social Security number (SSN) for all covered dependents. Healthcare Reform law requires the company to report this information to the IRS each year to show that you and your dependents have medical coverage and are not subject to a federal tax penalty. This information will be securely submitted to the IRS and will remain confidential.
When Coverage Begins You will receive an email from Plansource when it is time to make your elections. To be covered, you must complete the enrollment process by the deadline indicated in the email. If you enroll on time, coverage is effective on the first of the month following 60 days from your date of hire for non-exempt employees, and first of the month following date of hire for exempt employees. If you fail to enroll on time, you will NOT have benefits coverage (except for Company-paid benefits). Changes made during Open Enrollment are effective at the start of the plan year.

Be prepared to show documentation of the event such as a marriage license, birth certificate, or a divorce decree. If changes are not submitted on time, you must wait until the next Open Enrollment period to make your election changes.

Medical Plans GoodLeap is proud to offer you a variety of medical plans through Kaiser Permanente (California only) and United Healthcare PPO administered by UMR. Coverage under all plans includes comprehensive medical care and prescription drug coverage. The plans also offer many resources and tools to help you maintain a healthy lifestyle. Following is a brief description of each plan.
HMO (CA only) With the HMO plan, you select a Primary Care Physician (PCP) from the participating network of providers who will coordinate your healthcare needs including referrals to specialists and approving further medical treatment. Services received outside of the HMO are not covered, except in the case of emergency medical care.
PPO The PPO plan gives you the freedom to seek care from the provider of your choice. However, you will maximize your benefits and reduce your out-of-pocket costs if you choose a provider who participates in the United Healthcare network. The calendar year deductible must be met before certain services are covered.
HSA An HSA plan is a High Deductible Health Plan (HDHP) that gives you the freedom to seek care from the provider of your choice. However, you will maximize your benefits and reduce your out-of-pocket costs if you choose a provider who participates in the Kaiser Permanente or United Healthcare networks. The highlight of this plan is that it allows you to open a Health Savings Account (HSA) through PayFlex, which is a special savings account that allows you to save pre-tax dollars to pay for any “qualified healthrelated expenses” permitted under federal tax law.* This includes most medical care and services, prescription drugs, dental services, vision care, and expenses related to meeting the plan’s deductible. The money in the HSA is yours and it grows and remains with you, even when you change medical plans, employers or retire. Funds may be rolled over year to year and there is no “use-it or lose-it” rule. You decide when to use your savings to pay for qualified health-related expenses. This provides a strong incentive for you to spend wisely on your medical care, just like you do on other items you purchase. You may use your HSA funds to pay for qualified health-related expenses for yourself, your spouse, your tax-dependent children, and others you claim as dependents on your federal tax return — this is true even if they are not covered under the medical plan.

Coinsurance – Once you’ve met the plan’s annual deductible, you are responsible for a portion of your medical expenses, which is called coinsurance.
All medical plan claims must first be processed by the group health plan before the HRA Administrator, Grenz & Company, can review them for reimbursement. For this reason, all claims must be submitted with the Kaiser explanation of benefits (EOB). Additional provider or claim information may be requested to complete the process.

Health Savings Account (HSA) – To help offset the annual deductible, you may deposit your own dollars into your HSA through pre-tax payroll deductions.
The guidelines for contribution maximums are set by the IRS each year. The maximum amount that can be contributed into an HSA is outlined in the following table:
The HRA comes with a debit card to cover prescriptions and any services Kaiser applies to the plan deductible (capped at $250/ transaction). Kaiser will apply the plan deductible to everything except Preventive care and the first 3 office visits per year so the debit card provides easy access to HRA funds. All other HRA claims must be submitted to the HRA Administrator with a signed claim form, via the online portal or their mobile app. Reimbursements are processed within 7-10 business days. To improve reimbursement time, submit a onetime direct deposit form (all forms can be downloaded from the online benefits site). NOTE: HRA funds replenish as your deductible resets every January 1st. 2022 claims must be submitted by March 31, 2023 to be reimbursed (based on date of service, not payment date).

Your Pharmacy Plan GoodLeap partners with OptumRx to provide the prescription portion of the health plan (excludes Kaiser). The prescription program has three parts: Short Term Prescriptions You can get up to a 31-day supply of medication each time you have a prescription filled at a participating retail pharmacy. Ask your doctor or other prescriber to write a prescription for up to a 31-day supply plus refills, when clinically appropriate. The OptumRx Retail Program includes thousands of participating pharmacies nationwide. For a full listing, visit www.optumrx.com. Long Term Prescriptions GoodLeap has a cost saving mail order pharmacy option for long term prescriptions. The OptumRx mail order pharmacy provides convenient home delivery service for maintenance medications. Request a 90-day prescription from your doctor and the mail order pharmacy will fill it at a reduced cost compared to retail. Some retail pharmacies are also able to fill 90-day maintenance prescriptions but there you will be charged the full 3 month cost share. Specialty Medications Specialty medications must be ordered through Optum Specialty Pharmacy at 1-800-850-9122. Specialty drugs are limited to a 31-day supply. Prior authorization and quantity limits may apply. Specialty medications are subject to retail copays.
Additional Features, Reminders and FAQ CCS Medical LivingConnected Diabetes Management Program: Diabetic supplies are provided as part of the Prevention & Treatment Plan for diabetes and are provided to covered employees and their dependents at no cost to the employee (prescription copay is waived) when received from CCS Medical. These supplies include: cellular glucose meter, diabetic testing strips, control solution, lancets, lancing device(s), and alcohol pads. You can call 1-800-966-2046 to enroll in this voluntary program. Diabetic supplies not received from CCS Medical are subject to the plan copays. The OptumRx Mobile App: The OptumRx app makes the online pharmacy experience as simple as possible. It provides access at your fingertips to mail order prescriptions, refills, drug pricing information, prescription claims history, order status and much more. To get started, simply register at www.optumrx.com and download the free app on your phone or device. Generic drugs are another cost savings option built into your plan. Always request a generic medication from your doctor or pharmacist, if available and when appropriate for your medical treatment. HSA plan drug coverage: When enrolled in the HSA PPO plan, eligible prescriptions are subject to the medical plan deductible. Once the deductible is met, you will be responsible for the appropriate copay. There are some medications in the “preventive” category of coverage that will not be subject to the deductible. What is a Drug List or Formulary? Your plan uses what is called a “Preferred Formulary” that we also refer to as the “drug list” or just the “formulary.” This drug list/formulary contains brandname and generic medications approved by the Food & Drug Administration (FDA) that have been reviewed and recommended by your plan’s Pharmacy and Therapeutics (P&T) Committee. The P&T Committee is an independent group of practicing doctors, pharmacists, and other healthcare professionals responsible for the research and decisions surrounding our drug list. This

group meets regularly to review new and existing drugs and chooses the medications for our drug list - based on various factors, including their safety, effectiveness, and value. If your doctor prescribes a drug that is not listed on the drug list, you may be subject to extra out-of-pocket costs. Because the medications on the drug list/formulary are subject to periodic review, you may call Prescription Drug Member Services at 1-800-334-8134 to confirm if medications are included. You can also access a copy of the current list by logging into your OptumRx portal. What if my medication is not on the Formulary? If a drug your doctor prescribes is not on our drug list/formulary, please talk with your doctor about prescribing a medication that is included when appropriate. If a medication is selected that is not on your drug list/formulary, you will be responsible for the applicable non-formulary cost share amount. What is a prior authorization? Certain prescription drugs (or the prescribed quantity of a drug) may require a “prior authorization” before you can fill the prescription. Some drugs require prior authorization because they may not be appropriate for every patient or may cause side effects. Your doctor should have a current list of drugs requiring prior authorization. However, your doctor may call your plan’s referral number for authorization and information regarding these requirements. Prior authorization helps promote appropriate utilization and enforcement guidelines for prescription drug benefit coverage. What do I need to do if one of my prescriptions requires a prior authorization? Your doctor should have a current list of drugs requiring prior authorization. When you fill your prescription at a retail pharmacy, your pharmacist will be notified that your medication requires prior authorization and will take the necessary steps to request it. If you use home delivery, your doctor must obtain prior authorization before you can fill your prescription.

Flexible Spending Accounts (FSAs) GoodLeap provides you with an opportunity to participate in four different Flexible Spending Accounts (FSAs) administered through PayFlex. FSAs allow you to set aside a portion of your income, before taxes, to pay for qualified healthcare and/or dependent care expenses. Because that portion of your income is not taxed, you pay less in federal income, Social Security and Medicare taxes.
Healthcare FSA For 2022, you may contribute up to $2,850 to cover eligible healthcare expenses incurred by you, your spouse, and your children up to age 26. Eligible expenses include: Coinsurance Prescriptions Eye Exams/Eyeglasses Copayments Dental Treatment Lasik Eye Surgery Deductibles Orthodontia For a complete list of eligible healthcare expenses, visit www.irs.gov/pub/irs-pdf/p502.pdf. NOTE: If you enroll in the HSA medical plan, you may only participate in the Limited Purpose Healthcare FSA. This type of FSA allows you to be reimbursed for eligible dental, orthodontia, and vision expenses while preserving your HSA funds for eligible medical expenses.
Dependent Care FSA For 2022, you may contribute up to $5,000 (per family) to cover eligible dependent care expenses ($2,500 if you and your spouse file separate tax returns). Eligible expenses include: Care of a dependent child under the age of 13 by babysitters, nursery schools, pre-school or daycare centers. Care of a household member who is physically or mentally incapable of caring for him/herself and qualifies as your federal tax dependent. Commuter Benefits1 Commuter Benefits enable you to set aside money to pay for qualified workrelated mass transit and/or parking expenses on a pre-tax basis — reducing your taxable income. Your contributions are deducted from your paycheck each pay period. Qualified transportation expenses must be expenses incurred for you to commute between your place of residence and normal place of work. For 2022, you may contribute up to $280 per month into each account (transit and parking). This amount may change monthly, per IRS regulations. Exclusions apply. See plan document for details and exclusions.
IMPORTANT FSA RULES Because FSAs can give you a significant tax advantage, they must be administered according to specific IRS rules: Unused funds will NOT be returned to you or carried over to the following year. You can incur expenses through March 31, 2023, and must file claims by June 30, 2023. 1.
Life/AD&D Insurance Life Insurance provides your named beneficiary(ies) with a benefit in the event of your death. Accidental Death and Dismemberment (AD&D) Insurance provides specified benefits to you in the event of a covered accidental bodily injury that directly causes dismemberment (i.e., the loss of a hand, foot, or eye).
