Quality and Safe Care Newsletter Welcome to the October Edition of the Quality and Safecare Newsletter. You may be wondering where the Newsletter has been the last couple of months . A decision was made to make the Newsletter bimonthly to reduce the amount of publication staff have to read on a monthly basis. Unfortunately due to a lack of articles last month the newsletter was not produced. So again a warm welcome to this bumper addition. FLU Jabs Autumns arrived and Flu season will be upon us. Please check Sharepoint for dates when the flu jabs are being given within your area. In order to keep ourselves and the patient’s we care for safe, we are encouraging as many staff as possible to have the FLU Jab. Some staff may have this with their GP practice and this is fine but let your manager know if you have had it through your GP. Community Gulp Assessment Screen shots and information
Introduction to the New Matrons within the Quality Directorate. Early this year Sue Dakin retired, and the successful candidate to take over the role and become the Matron for Quality Specialist Services was Julie Mills. Julie in her previous role within DCHS was our lead for patient manual handling and bariatric care. Julie will oversee the following specialist teams; Infection Prevention & Control, Falls, Patient manual handling, Bariatric care and Diabetes. Trish Mahon Matron for Tissue Viability retired in September. The successful candidate to take over the role is Jeni Townsend who’s previous role within DCHS was clinical lead for Tissue viability.
Community Gulp Assessment was changed in July 2018 and the question for urinalysis was removed. This communication is just a quick reminder and to include GULP Screenshots the screen shots and information of completion. We wish you both a successful career in your new roles. Patient Experience update
Patient Experience Team (PET) Patient Opinion is a non-profit service which has helped resolve issues, restore relationships, encourage staff and improve services in many parts of the UK. As health and social care services across the UK continue to evolve and integrate, Patient Opinion and Care Opinion are merging their two services into a single integrated and simplified service. One online feedback platform will handle stories from everyone, whether they are about health or social care services. The new name will reflect this unified approach: Care Opinion. New features and developments • Data visualisations for exploring story themes – There was plenty of interest in the innovative tag bubble visualisation that was released last summer and Patient Opinion have continued to develop these alongside other data visualisations to help staff monitor and compare performance across service areas and themes. Explore a live interactive example • Story author demographics –To learn more about the people sharing their stories on Patient Opinion with the new demographics report. More information on our blog. • Making online feedback dementia-friendly - The partnership with Talking Mats will make it easier for people with dementia (or other cognitive impairment) to share their story on Patient Opinion. The first stage testing this will be in April. See progress so far • Subscriber digests - As of 1 January, Patient Opinion offer a weekly digest email to tell subscribers what’s been happening with their subscription. This is available at all subscription levels and is a great way for them to keep an eye on developments.
From 1 May onwards, everyone visiting Patient Opinion will be automatically redirected to the new Care Opinion site. Although the site will have a new name and address, it will look more or less just as it does now, and everything will continue to work in the same way. All existing leaflets, posters etc can continue to be used until new ones are needed and new materials are being created with a fresh new logo to match the new identity. I have kept this article in this Months newsletter due to some confusion in the ReSPECT form .
What is ReSPECT? ReSPECT stands for Recommended Summary Plan for Emergency Care and Treatment. The ReSPECT process creates a summary of personalised recommendations for a person’s clinical care in a future emergency in which they do not have capacity to make or express choices. Such emergencies may include death or cardiac arrest, but are not limited to those events. The process is intended to respect both patient preferences and clinical judgement. The agreed realistic clinical recommendations that are recorded include a recommendation on whether or not CPR should be attempted if the person’s heart and breathing stop. How does it work? The plan is created through conversations between a person and one or more of the health professionals who are involved with their care. The plan should stay with the person and be available immediately to health and care professionals faced with making immediate decisions in an emergency in which the person themselves has lost capacity to participate in making those decisions. ReSPECT will be used across all health and care settings within Derbyshire, including the person’s own home, an ambulance, a care home, a hospice or a hospital. Professionals such as ambulance crews, out-of-hours doctors, care home staff and hospital staff will be better able to make immediate decisions about a person’s emergency care and treatment if they have prompt access to agreed clinical recommendations on a ReSPECT form. How will ReSPECT be implemented? ReSPECT will be implemented across Derbyshire’s health and social care communities in June 2018. The ReSPECT form will replace the Unified DNACPR form utilised within Derbyshire and will also replace existing DNACPR forms used within Royal Derby and Chesterfield Royal Hospitals. The ReSPECT form will not replace the Health and Social Care Summary, but will summarise any decisions made about emergency care and treatment, which may include CPR decisions. A Task and Finish Group has been formed within DCHS to ensure that the ReSPECT process is embedded within the organisation, to raise awareness of the process amongst staff and the public, to ensure that relevant staff members have access to specific training where required and to monitor and review the change in practice after June.
