Assessment 2 – Risk Assessment and Participate in a Work Health and Safety Meeting Assessor Instructions: This assessment is broken up into three (3) tasks: • • •

Task 1 requires the student to conduct a workplace risk assessment. Task 2 requires the student to participate in a simulated workplace WHS meeting. Task 3 requires to the student to participate in a simulated debriefing session.

Task 1 This assessment requires the student to conduct a formal workplace risk assessment and record the results. The assessment can be conducted in a simulated or real work environment. Clear instructions are included within the assessment to simulate realistic in-depth scenarios if the assessor chooses to conduct the assessment in a simulated work environment.

Simulated work environment assessment If assessment is to occur in a simulated work environment, the assessor is to ‘set up’ multiple hazards for students to identify. The following list gives example hazards that can be ‘set up’ by the assessor. • Electrical leads across walkways (trip hazard) • Water spilled on floor with yellow food colouring to simulate urine (slip hazard, infection hazard) • Contaminated linen on the ground (trip hazard, infection hazard) • Blocked emergency exits (fire hazard) • Broken equipment (faulty equipment hazard) • Debris on floor (slip or trip hazard) • Incorrect storage of chemicals (chemical or fire hazard) Real work environment assessment If assessment is to occur in a real work environment, the student is to conduct the risk assessment in the workplace, identifying real hazards.

Students are to follow the instructions in the assessment and identify (2) hazards in the work environment.

Students are to complete the Risk Assessment Form (the form can be used for both hazard identifications).

Students are not required to complete step 3 of the risk assessment form – Control the Hazard, until they participate in the workplace safety meeting in Task 2.

Task 2 This assessment requires the student to participate in a simulated workplace safety meeting. All instructions for Task 2 in this assessment must be followed, and Observation Form 1 – Part 1 completed by the assessor while the student is participating in the workplace safety meeting. 1.

Students are required to be broken up into groups to participate in a simulated workplace safety meeting.

The assessor is to act as the meeting chairperson or allocate one of the students to be the chairperson. The chair person must ask the oral observation questions (provided in Task 2) at the meeting. Students are required to work as a team to come up with solutions to the WHS hazards.

Follow all instructions in the Task 2 section of this assessment to identify the topics to discuss in the meeting.

In the workplace safety meeting, students are required to: • Raise hazards identified in Task 1 using positive communication. • Discuss solutions to control the hazards identified in Task 1. • Listen to identified hazards raised by colleagues in the workplace safety meeting. • Give constructive feedback and opinions to develop and implement safe work practices.

Assessor is to complete Observation Form 1 – Part 1 for each student that is participating in the meeting.

Task 3 This assessment requires the student to participate in a simulated debriefing session that should be carried out after the workplace safety meeting has been concluded. Students are to stay in the same groups to complete Task 3. All instructions for Task 3 in this assessment must be followed, and Observation Form 1 – Part 2 completed by the assessor while the student is participating in the debriefing session. 1.

In the debriefing session, students are required to: • Discuss and reflect on stress and fatigue levels (individual needs). • Consider ways to address their individual needs.

Assessor is to complete Observation Form 1 – Part 2 for each student that is participating in the meeting. This is NOT a group assessment. Each student is required to have a separate observation form filled in.

Task 1 Conduct a Risk Assessment 1.

Read the Safe Work Policy and Procedures in the Policies and Procedures Manual and write down the procedures for carrying out a formal risk assessment. If your workplace has their own policies and procedures, use these instead of the Support by SDL procedures.  A formal risk assessment is to be carried out when a hazard is identified and a staff member with the appropriate authority, such as operations manager or clinical manager, has requested a formal assessment to be carried out  Complete the work place Risk Assessment Form and submit to your supervisor.

Instructions for Risk Assessment Your supervisor has asked you to perform a formal risk assessment in your workplace. You are required to follow your workplace Policies and Procedures when carrying out the risk assessment. You are to identify two (2) hazards in your workplace and complete the Risk Assessment Form provided (the form can be used for both hazards). You are not required to complete Step 3 – Control the risk section of the form until you participate in the workplace safety meeting in Task 2 of this assessment. 2. Write down a description of each hazard below. Hazard 1 Answers will vary, but examples are:  Fire exit blocked with furniture creating a fire hazard.  Urine spill on ground creating a slip, trip and fall hazard and an infection hazard.  Electrical lead is running across walkway, creating a slip, trip and fall hazard. Complete the risk assessment form on the following page for the two (2) hazards identified. Do not complete Step 3 of the form until the WHS consultation meeting.
